Patient Portal LoginIf you are a current patient and have activated your portal during check in, you may use the button above to access and update portions of your medical record. For new patients that have a pending appointment scheduled, you may fill out details about your medical history and other pertinent information to speed the process of check in during your first visit. Please call the office and speak with a receptionist to have your portal activated. You must have a valid email address in order to set up a secure patient portal password.

New Patient Appointments
We look forward to serving you. To ensure that your visit goes smoothly, we request that you bring the following with you to your appointment:
- • Photo identification
- • List of current medications
- • Insurance card(s)
- • Please be prepared to pay any applicable deductibles, copays, or coinsurance at the time of your visit. We accept cash, check, and major credit cards.
We request that you arrive approximately 15-20 minutes early to complete paperwork. For your convenience, you may complete your registration forms (see link above) prior to your appointment and bring them in with you. In addition, you may choose to fill in your medical and other history in your chart. A link to your patient portal can be generated at the time of scheduling if desired.
Our office uses an automatic system that utilizes voice, text, and email reminders to reduce missed appointments. Please respond to this system or call if you are unable to keep your appointment, and our office will be happy to reschedule your appointment to a more convenient time. We require at least 24 hours notice to avoid being assessed a no-show fee.
